Who pays for the treatment?
In most cases, health insurance will cover your treatment. We take virtually all insurances with few exceptions. Make sure you talk to our billing representative to help you clarify your insurance coverage.

What is a co-pay?
 Some insurance contracts require you to pay a certain amount each visit. This amount is called a co-pay. Co-pay amounts can vary depending on your individual policy. Co-pays are expected to be paid on your arrival each visit.

What is a deductible?
The deductible is the amount you are required to pay before your insurance begins payment. In most cases, when you see practitioners that are “in network”, there is no deductible. Deductibles vary depending on your individual policy. You should check with your insurance company to determine your individual plan coverage. If you have a deductible and have not met it, payment is expected each visit until your deductible is met.

What is co-insurance?
Your responsibility (at the time of service) as determined by certain health insurance plans. Typically a percentage of the charge allowed by your insurance plan (i.e. 10%, 20%).

How do I pay for my co-pay, co-insurance or deductible?
Co-pays are due at the time of service. We accept most major credit cards, cash or personal checks.

Cancellations/No Show Fees

We understand the scheduling challenges that physical therapy can pose to you. It often requires a substantial time commitment and can be disruptive to your daily routines. We will do our best to be prompt to each and every appointment during your care. In return, we ask that you review our cancellation policy very carefully and be aware that we will impose a penalty fee of $25.00 for each appointment that is missed without proper notification. You can avoid this fee by simply notifying our office 24 business hours before cancelling a scheduled appointment. Our office will try to call to confirm appointments the day before. This is a courtesy call, and you will still be charged a fee if your appointment is missed and you did not receive this call.

Medical Records

There are situations when you have Physical Therapy that your Primary Care Physician or other Specialist(s) may need information regarding your treatment in order to make informed recommendations about your treatment of care. Copies of pertinent medical records are available to the patient or an authorized representative for a nominal fee after a Release of Information form has been completed.  Total Health Center, Inc.will be more than happy to provide the needed documentation to assist in your care. However, we want to be sure that your privacy is protected and that your information is only sent to those authorized by you.
